Can I use Multiple Image Upload and add to DB with existing Extension form, also can have it so if I say had 5 images to upload I could add to DB as one record with fields image1, image2, image3, image4, image5 for example ?

Sorry, what are you trying to accomplish? Each image has a database record, they are not combined.
