Even when I untick 'Allow Comments' on an album, and on each image within it, th form still shows up... any ideas? Is there anywhere else in admin I need to change the settings on? It's a customised theme - could that be anything to do with it?

Have you perhaps modified the theme? There needs to be an "if" statement around the comments block in image.php that reads if (getOption('Allow_comments')) {
